Continuum of Care
Continuum of Care is an integrated program of treatment services and therapies designed to meet your individual needs as you advance through the recovery process. The array and intensity of services will reflect the severity of your condition. Individual progress is continually assessed at each level of care.
The levels that make up our Continuum of Care model are based on the Patient Placement Criteria established by the American Society of Addiction Medicine. You will enter the program at a level that reflects your individual needs.
These levels are as follows:
Level II.1 - Intensive Outpatient
Level III.2D - Sub-Acute Detoxification

Continuing Care
Continuing Care is essential to an individual’s success in the program. It reinforces the therapy received during treatment and it is the basis for relapse prevention and on-going recovery. Because relapse is a process, not a single event, Continuing Care provides the support and the tools necessary to identify warning signs of a potential relapse and the opportunity to take action before an actual relapse occurs.
